Understanding Alcoholism Stages: Changes in Character and Behavior

In the progression from early to more advanced alcoholism, changes occur in the person’s character under the influence of alcohol. The middle stage, in particular, is marked by irritability, mood swings, and unusually intense emotional reactions, a psychologist and addiction specialist at a leading Russian medical university notes. This view reflects observations from experts working within the country’s health system.

“During the middle stage, there is a noticeable shift in character. For instance, previously pleasant emotions, a sense of joy, and a feeling of being oneself while not drinking may give way to irritability, ongoing mood fluctuations, and disproportionately strong emotional responses both when alcohol is consumed and, more strikingly, when it is not,” the specialist explains.

Additionally, the middle stage features a diminished capacity for self-criticism regarding one’s situation. The person with an alcohol use disorder may no longer recognize the illness or acknowledge the need for change, even when relatives offer guidance. In the early stage, listening to advice is more feasible, but such receptivity often fades as the condition progresses.

“In the third stage, changes in character continue. Cynicism, aggression, cruelty, frequent conflicts, resentment, and social withdrawal become more pronounced. The person may appear more primitive, lose the ability to appreciate humor, and experience heightened irritation under mental stress. Healthy interests tend to disappear, signaling a decline in cognitive and social functioning,” explains a narcologist who specializes in addiction medicine.

Experts emphasize that recognizing the earliest signs of alcohol-related problems is essential for timely intervention, and begin treatment in the initial stages to improve outcomes and reduce long-term harm.
