Doctors will be able to remotely monitor the health of Muscovites with chronic diseases

Using proactive dispensary dynamic monitoring, Moscow doctors will be able to monitor the condition of about one million Muscovitees with serious chronic diseases online. This was announced by the Deputy Mayor of Moscow Social Development Anastasia Rakova.

A new section in the electronic medical record called “My Self-Regulatory Questionnaire” has appeared, where patients can enter data about their health status.

Doctors will monitor their dynamics online and, if necessary, invite citizens to an unscheduled appointment to arrange additional examinations or therapy.

According to Rakova, the proactive dispensary dynamic monitoring project started working in all adult city outpatient clinics as of this year.

“We conducted a special self-control questionnaire on electronic medical records with a convenient automatic reminder system. Physician assistants will teach patients how to use this service. In addition, personalized questionnaires will be conducted for patients under dispensary treatment, taking into account a specific disease. Alarm alerts are issued to doctors if a person has deviations from the self-control questionnaire, for example, an increase in blood pressure, ”said the deputy mayor.

The information entered by patients allows doctors to evaluate the effectiveness of the prescribed treatment, as well as adjust the treatment regimen and recommend additional studies.

Patients can fill out the self-control questionnaire in a separate section with the same name, or they can continue to fill it out from the “My Health Diary” section. The list of questions is created according to the patient’s disease. Smart notifications remind you to enter data.

400 paramedics are in contact with Muscovites, their task is to involve them in taking care of their health, helping to form a responsible attitude to medical advice and prescriptions, as well as helping to organize appointments and examinations. If necessary, their number will be increased to 600 experts.

The results of the analysis of the main causes of death at home formed the basis of the list of nosologies requiring dispensary observation. The program included common diseases such as type II diabetes mellitus, hypercholesterolemia, arterial hypertension, coronary heart disease and myocardial infarction, atrial fibrillation and flutter, chronic heart failure, acute cerebrovascular accident, chronic obstructive pulmonary disease, peptic ulcer disease.
