Modder cnfilmm restored Vesemir’s appearance, as seen in the early concept art of The Witcher 3. You can download the mod here (it weighs about 50MB).

A new mod called Concept Vesemir changes the witcher’s armor and face, making him look older and shabby. The enthusiast did not change the shape of the face itself, only changed its texture.
